Our NHS Referral Process
1. The NHS Health professional first completes the referral form. To obtain a referral form, please request this by emailing: referrals@wessexdriveability.org.uk or by calling us on 02380 554 100. Our Centre will email or post referral forms to NHS professionals only.
2. Once we have received the referral, we will post an application pack to the individual. We ask that on all occasions, the patient has already been made aware that Wessex DriveAbility will be in contact.
3. Once we are in receipt of a completed application form and payment, we will book the assessment date.
4. Once the assessment is completed, we will post the individual and the referring health professional a copy of the assessment report, and if appropriate, we will also ask the individual to post a copy of the assessment report to the DVLA.
Should the individual choose not to have the assessment or we have not received a response, following 21 days after we have posted the information to the individual, we will destroy the referral and the referring health professional will be informed.
