APPLICATION FORM FOR AN ASSESSMENTHow to Apply for a Driving or Transfer Assessment
There are a number of ways to apply for an assessment:
- Call 023 8051 2222 to request an application form to be sent to you by email or post.
- Select a form from the two options below to print off and post to us. (requires MS Word)
- Complete our 'Online Application Form' below. When completed please ensure you select 'Submit' at the bottom of the form.
If you are unsure as to which assessment you require, either visit the 'Assessments' page, or contact the centre.

Until March 2012 all Driving and Transfer Assessments are FREE OF CHARGE! Please quote WD1.
CancellationsIf for any reason you are unable to attend your appointment, please contact the centre on 023 8051 2222 as soon as possible. Please note that in the event of you cancelling the appointment at short notice and without good reason, no refund of your fee will be made unless we can reallocate the appointment. In which case, a refund will be made to the amount of your payment, less £15 retained to cover administration costs.
